DEDUCTIBLE
Amount You Pay During The Coverage Period For Health Care Services Before Your Plan Pay.
CO-INSURANCE
Your Share Of The Costs Of A Covered Health Care Service Calculated As A Percentage. Most Plans Require You To First Meet The Deductible Before Coinsurance Is Applied.
COPAYMENT
Fixed Amount Paid For A Covered Health Care Service Made At The Time The Service Is Provided.
PREMIUM
Amount Paid For The Health Insurance Plan To Be Active. Typically Paid Yearly Or Monthly
OFF-EXCHANGE PLANS
Off-Exchange Refers To An Enrollment That Is Done Directly With An Insurance Carrier And Does Not Take Place On The Federal Marketplace. These Enrollments Do Not Receive A Subsidy But Also Do Not Require Personal Information.
ON EXCHANGE PLANS
On-Exchange Refers To An Enrollment That Is Done Directly On The Federal/State Marketplace. These Plans Are Eligible For A Subsidy. They Do Require Proof Of US Citizenship, Residency, Or Other Qualifying Immigration Status.
SUMMARY OF BENEFITS & COVERAGE (SBC)
Provides Summary Information On The Plan Costs (In & Out Of Network) For All Covered Services And Medical Events. A Variety Of Common Medical Events.
