By becoming a volunteer at Crosby Health Centers, I acknowledge and agree to absolve Crosby Health Centers of any responsibility for injuries or accidents that may arise during my participation in any sponsored function, program, or activity.

By this agreement, I release Crosby Health Centers from any liability related to the use of my likeness. I grant permission for Crosby Health Centers to utilize my likeness in various materials, including newsletters, educational materials, and other printed materials concerning the organization. Additionally, I consent to the release of information about me by associates of Crosby Health Centers to clients and other volunteers as needed to support my volunteer activities.

As a volunteer with Crosby Health Centers, I commit to attending all mandatory trainings and meetings, completing all necessary paperwork, and fulfilling the expectations of my role to the best of my abilities. I will honor all commitments, adhere to punctuality, and promptly communicate any scheduling conflicts to my immediate supervisor.

I acknowledge that this agreement will remain in effect for one year from the date of this application and will automatically renew each year on the anniversary of my joining Crosby Health Centers, provided that I maintain active volunteer status according to the organization's criteria.
