Patient acknowledges that Crosby Health Center mental health and/or addiction services are provided by licensed clinicians and interns/ master level professionals working under the supervision of licensed clinicians.

Patient acknowledges that appropriate referrals will be provided for behavioral health services outside of the scope what Crosby Health Center offers (i.e. disability assessments, child custody assessments)

Patient acknowledges that he/she/they are responsible for providing eligibility documentation (proof of income, residency, HIV status, primary care, and/or insurance) within 30 days of enrollment for services.

Patient acknowledges that Crosby Health Center does not offer stand-alone psychiatric services

Patient acknowledges that barriers to care (transportation, childcare, technology, schedule conflicts) will need to be addressed with therapist to assist with scheduling and maintaining appointments
